ElevateMind: Your Partner in Mental Health

ElevateMind, LLC is a practice dedicated to providing a comfortable and safe space that builds connection, promotes health, provides solutions, and empowers others to understand their significance. EM continually strives to provide well-studied and evidenced-based intervention and evaluation approaches in psychology, education, and mental health to consumers of our practice. Our assessment and evaluation services follow a neurocognitive process-oriented approach, which integrates quantitative test data, qualitative behavior observations, background information, and current research in psychology and neuroscience. Our intervention services use a number of evidenced based approaches that have been shown to be effective in treating mental health conditions, including Depression, Anxiety, Anger, Trauma, OCD, ADHD, etc. 

All fees at ElevateMind, LLC are established as an hourly or per session rate and all services are billed on the basis of time. This includes diagnostic interviewing and testing, psychotherapy, preparation of letters and reports, consultations, and travel time for off-site services. Our current Fee Schedule is available in our office’s waiting area or you may contact our office for specific rates. All private pays, copays, and coinsurance are due at the time of service.

ElevateMind, LLC is in-network with a number of insurance companies; however, we are not in-network with all insurance plans. In addition, clinicians of the practice may differ in their insurance network affiliations. We suggest that you contact your insurance company prior to the first appointment to obtain specifics about your insurance coverage.

If we are not in-network with your specific insurance plan, you should contact your insurance provider to check if you have any out-of-network benefits. If so, you may receive reimbursement by your insurance company, even if you have to pay out of pocket at first. Many people have found this to be a good strategy for finding a provider of their choosing, rather than being forced to stay within their insurance network.

Note: Testing for education-related concerns (e.g., Learning Disabilities, etc.) and for specialized issues (e.g., Act 235 evaluations) is not covered by most insurance plan, and thus, patients are responsible for the cost of these services.

For your convenience, ElevateMind, LLC accepts cash, personal check, MasterCard, Visa, and Discover. Returned checks will result in an additional $35.00 fee to cover bank charges and processing costs. If you are not able to pay your copay, coinsurance, or deductible on the date of service, your appointment must be rescheduled, unless prior arrangements have been made.

For those patients without any insurance coverage and who are in financial need, ElevateMind, LLC will provide services on sliding scale or at a reduced fee. This is not available for persons who have insurance and could receive service from another provider. Reduced fees are offered on a case-by-case basis, and are limited to availability. To request a fee reduction, potential patients must complete a Fee Reduction Request Form, and it will be reviewed. All final approvals are made by the Office Manager.
